April 30, 2024 BACKGROUND CHECK AND DRUG SCREENING IS A CONDITION OF EMPLOYMENT The Health and Human Services Coordinator plays a pivotal role in facilitating access to healthcare services and improving health outcomes within Gadsden County.
This position involves coordinating various healthcare initiatives, programs, and resources to address the unique needs of the community.
The Health and Human Services Coordinator works closely with healthcare providers, community organizations, and government agencies to ensure the efficient delivery of healthcare services and promote wellness initiatives.
Essential Functions:
Coordination of Healthcare Services:
Collaborate with healthcare providers, including hospitals, clinics, and practitioners, to coordinate healthcare services for residents of Gadsden County.
Serve as the Liaison for the Gadsden County Health Department and the Gadsden County Health Council.
Community Outreach:
Engage with community members to identify healthcare needs, raise awareness about available services, and promote health education initiatives.
Resource Management:
Manage resources, including funding, grants, and healthcare facilities, to optimize healthcare delivery and address gaps in service.

Patient Advocacy:
Advocate for patients' rights and ensure they receive quality care by assisting with navigating the healthcare system, addressing barriers to access, and facilitating communication between patients and healthcare providers.
Data Analysis and Reporting:
Collect and analyze healthcare data to identify trends, assess the effectiveness of healthcare programs, and generate reports for stakeholders.
Monitor programs funded by the Gadsden County Board of County Commissioners.
Collaboration and Networking:
Build and maintain relationships with healthcare stakeholders, community organizations, government agencies, and other relevant partners to foster collaboration and support healthcare initiatives.
Program Development:
Assist in the development and implementation of healthcare programs and initiatives tailored to the specific needs of Gadsden County residents.
Develop and implement the Opioid program.
Health Promotion:
Develop and implement strategies to promote preventive care, healthy lifestyle choices, and disease prevention within the community.
KNOWLEDGE, SKILLS AND ABILITIES:
Ability to initiate and maintain positive working relationships with the healthcare community, the citizens of Gadsden County, and other organizations.
Knowledge of some medical terminology required.
Ability to communicate effectively before a group of individuals.
Ability to multi-task and work independently.
Knowledgeable of Microsoft Office Suite.
Knowledgeable of HIPPA requirements and guidelines.
Strong meeting and group facilitation skills.

Qualifications:
Bachelor's degree in public health, healthcare administration, social work, or a related field (Master's degree preferred).
Years of experience can substitute for formal education or qualifications.
Experience working in healthcare coordination, community health, or a related field.

Strong communication, interpersonal, and organizational skills.
Ability to work collaboratively with diverse stakeholders and communities.
Proficiency in data analysis and report writing.
